Description

The Alliance of Colorado Community Health Workers, Patient Navigators and Promotores de Salud, in partnership with Patient Navigation Training Collaborative, presents the first in a series of webinars on barrier reduction.  The May 28 session will be on transportation, defining the issue and exploring opportunities to help with transportation assistance. Learn about strategies to help your patients get the transportation they need to medical care. The Colorado Health Institute, American Cancer Society, Touching Hearts of eastern Colorado, and more will share details about their work and potential resources for Colorado communities. 

Moderator: Andrea Dwyer of the Colorado School of Public Health on behalf of the Alliance.

Audience: Those working with patients to reduce barriers to care, including patient navigators, community health workers, care coordinators and promotores.

This webinar series is offered free of cost due to funding from the Colorado Health Foundation.

 

Presenter(s) and/or Content Experts

Jackie Zubrzycki is a communications specialist at the Colorado Health Institute and a former education reporter and teacher based in Denver. She will share data and trends about transportation and access to health care from the 2017 Colorado Health Access Survey.

 

 

 

Jen Briggs with American Cancer Society manages the Colorado Access to Care programs and Reach to Recovery for breast cancer patients. She will talk about the transportation services the American Cancer Society provides and moving towards a higher focus on Access to Care and community mobilization.

 

 

 

 

Tara Gaynor, owner of non-medical home care agency Touching Hearts at Home, will be presenting information about transportation in rural Colorado. Transportation for older adults and those with disabilities is definitely a problem in rural Colorado. 

 

 

 

 

 

 

Annette Weber is the coordinator of the East Central Council of Local Government (ECCOG) to help with transportation in eastern Colorado with their Outback Express. Annette will share experiences about access to transportation in rural Colorado.
