The goal of this course is to equip public health professionals with the information and skills they need to apply a systems thinking approach in order to:
Identify upstream determinants impacting specific health outcomes
Identify shared risk factors across multiple health outcomes
Identify and select particular upstream factors which they want to target
Describe metrics by which to measure changes in upstream health determinants
Identify strategies to impact those upstream determinants
Communicate and work with partners to identify common goals
Identify and implement collective funding approaches to support innovative efforts
Assess organizational readiness or barriers to authentically engage the community in a shared risk approach
Identify actionable steps to impact the root causes resulting in poor health outcomes in their communities
Work for the course will be completed together as a team in pairs or trios. Together, teams will take their understanding of social determinants of health to the next level as they identify actionable steps to impact the root causes resulting in poor health outcomes in their communities.
Application Information
In order for a team application to be reviewed and considered for this training, participants must have at least 1-2 other partners (up to groups no larger than 3 people) from within their department or organization to form a team. Once all partner(s) have been identified, submit one application per team with all partner(s) listed.
If you do not have a team, please still submit an application as we may be able to assist you in finding one. Please note however, if we cannot pair you with a team we will not be able to admit you into the course and encourage that you re-apply for consideration at a future opportunity.

Cost
No cost for registrants from our region (Colorado, Montana, North Dakota, South Dakota, Utah, Wyoming), $160 for registrants from all other states, no cost for registering for the waitlist - payment arrangement will be made by staff if spot becomes available.
