Identify and select particular upstream factors which they want to target
Describe metrics by which to measure changes in upstream health determinants
Identify strategies to impact those upstream determinants

In order for a team application to be reviewed and considered for this training, participants must have at least 1-2 other partners (up to groups no larger than 3 people) from within their department or organization to form a team. Once all partner(s) have been identified, submit one application per team with all partner(s) listed.
If you do not have a team, please still submit an application as we may be able to assist you in finding one. Please note however, if we cannot pair you with a team we will not be able to admit you into the course and encourage that you re-apply for consideration at a future opportunity.
Please review or download the application questions here if you'd like to work on your application offline as you must be able to submit your application information in one-sitting. When your team is ready to submit your team application, please click on this link to submit it.
