Description

This course is full, use code PNCT2023 to be added to the waitlist at no cost. 

This course focuses on the role of the health navigator in team-based care coordination. In this course, we will explore care coordination and related concepts, including their application to practice and commonly encountered challenges. You will be guided step-by-step to learn the basic skills and advanced strategies to enhance team-based care and client engagement for better health outcomes, patient safety, and workflow efficiency.

Audience

This course is designed for care coordinators, case managers, patient navigators, health navigators, community health workers and others who want to provide high quality, coordinated care to their clients.

  • An introduction to team roles and functions for effective care coordination 
  • Communication techniques for team responsibility and accountability to a shared plan of care
  • Tools to efficiently manage your patients panel  
  • Overview of strategies you can use with your clients to enhance self-care 
  • Ways to measure the effectiveness of care coordination 
  • Case-based discussion and guided practice  

Course format

This course is an instructor-led virtual course (not self-paced). The format include both online coursework (reading, assignments, quizzes) as well as Live Learning Sessions via Zoom web conferencing. These sessions are NOT a webinar, but a chance to connect with your peers and practice skills. The time commitment is 3-4 hours per week.

  • Describe the different roles & responsibilities of interdisciplinary care team members
  • Apply evidence-based care coordination skills to common population-based patient scenarios
  • Discuss ways to measure and report the effects of care coordination services

Please ensure you receive a confirmation email. If you do not receive a confirmation, your registration was not processed.

Refund and cancellation policy:

  • If you are unable to attend the course, you must cancel your registration at least 14 days (2 weeks) before the first day of course in order to receive a full refund.
  • If you cancel with less than 14 days (2 weeks) notice, you will be subject to a $75 cancellation fee.
  • To cancel, you must send an email to admin@patientnavigatortraining.org.
  • No-shows will not be granted a refund.
  • Refunds will be issued on the original card used for payment OR as a credit toward a future course. Refunds are subject to a $15 credit card processing fee.  

Please note: The PNTC requires a minimum number of students registered to hold a course. If the minimum enrollment is not reached 10 days prior to the course date, the course may be cancelled. Participants will be notified via email and registrations will be refunded in full.

Level 1: Health Navigation Fundamentals or experience in the field (such as RN, LPN, LSW or more than 2 years experience as a PN)
Technical Requirements
A webcam or access to an internet capable device equipped with a camera and a microphone (such as smartphone or tablet) is required to participate in this course.
